[ayso45-refs] Missing Team Cards

Generically speaking, it is the Coach who is fielding the team and
his/her responsibility to provide the ref's a lineup.
There have been lots of anecdotes about why it is a good idea for ref's
to have a back up plan by printing the card.
This week I had a U16B game where the coach had formed the team 4 days
prior and he didn't have access to WYS yet.

So let's reinforce the coach's role and have in our back pocket the
contigency plan.
